Here are some forms that will be helpful to us as we start. They can be printed and completed prior to your appointment. Please, read them carefully and if you have questions please don’t hesitate to ask.
* The Notice of Privacy Practices form is for you to read and keep for your records; you do not need to return that to me. The Privacy Practices Signature Page needs to be printed and signed, acknowledging that you have been given a copy of your privacy rights.
